The admission form must be completed in all respects with two passport-size photographs must be submitted along with the following documents:
a) A Provisional certificate issued by the Head of the institution last attended. The certificate must contain the following information in respect of the candidates
(i) Date of Birth (ii) The last examination passed.
b) Character Certificate.
